Palliative and End of Life Care Standard

Death and dying affects all of us, and the quality and accessibility of palliative and end of life care is vital to everyone.

The needs of people of all ages who are at or near the end of life, their families, carers and communities, should be addressed, taking into account their priorities, preferences and wishes. Personalised care as individuals near the end of life will result in a better experience, tailored around what really matters to the person, and create more sustainable NHS services.

PRSB has updated the SCCI 1580 standard for palliative and end of life care coordination to ensure that professionals and individuals have access to appropriate information to support decision making for those with palliative care needs and those who are approaching the end of their lives. The standard supports the Universal Principles for Advance Care Planning.


The standard

PRSB was commissioned by the NHS to produce the Palliative and End of Life Care Information Standard, otherwise known as the PEoLC Information Standard or EPaCCS Information Standard. PRSB worked with NHS colleagues, including a range of subject matter experts, professionals and PRSB member organisations representing the four nations of the UK, patient representative groups and the voluntary, charitable and social enterprise sectors.

The standard defines the information that should form part of a person’s shared care record for use whenever or wherever a person accesses services.

The standard applies to:

  • All services that support individuals receiving palliative or end of life care.
  • It was developed in consultation with a wide range of professionals from all four nations, including from professionals working in primary, secondary, community and third sector services involved in palliative and end of life care, IT suppliers and people who use services, and carers.
 

Implementation toolkit

A web based toolkit has been developed to support the adoption and implementation of the Palliative and End of Life Care Information Standard. The materials and resources in this toolkit will support you with your local implementation of the standard.

Provenance data

Provenance data for this standard will be shown in a separate information model in the next iteration of the standard following its 3-year maintenance release. Provenance data describes who made the record entry or carried out the activity, where and when. The latest provenance model is available on our website.
