Nourish Care – the first supplier to achieve conformance against the Urgent Transfer from Care Home to Hospital Standard

One of the biggest risks health and care professionals face is providing continuity of care when people move from one care setting to another, and that is why sharing relevant information is fundamental to safe, effective, high-quality care. Good handover information can help reduce time spent in hospital, support people to return home earlier and improve their outcomes. A case in point is when a person is transferred urgently from a care home to hospital. It is key to ensure that first response teams have all the information they need to make those transfers as smooth as possible.   

Nourish Care, a supplier of care management software, has successfully achieved conformance against the PRSB’s Urgent Transfer from Care Home to Hospital Standard, the first supplier to attain a Quality Mark for this standard.  

Having previously achieved conformance for About Me and the Personalised Care and Support Plan Standard, Nourish was familiar with our assessment process and found it easier to demonstrate compliance. With conformance against three standards, Nourish have established themselves as leaders in information standards and interoperability, demonstrating the quality of their products. They are keen to build on this success further and work towards conformance with our other standards.
