Take part in our survey
We are conducting two surveys aimed at helping the NHS and social care services to share information securely wherever and whenever it is needed for better care and support.
The first survey looks at the following information. Each of these sections has supporting documentation that outlines the content in full and a draft version of the standard.
- The information that people want to share about themselves – “About Me”

- The information that the local authority holds about people that would be useful for health and care professionals in providing care and support in hospital, at home or in a care home

- The information that care home staff can see when information is shared between health and social care services

We need your help to define the information that is needed in these areas of the health and social care record.
What will I need to do?
The survey is designed to be easy to complete. We will ask you for a few details about yourself and your role, this is so we can ensure you are directed to the right set of questions for you to answer. It should take about 15 minutes to complete the survey.

Easy read version
We have made an Easy read survey for the About me section. Easy read is for people who find it hard to read or cannot read but can understand spoken English. An easy read sentence only contains one idea, has no jargon or difficult words and is supported by a picture for each idea/sentence. Here is a helpful fact sheet to help with completing the survey.
