The Interweave technology was designed and built by the Yorkshire & Humber Care Record as part of NHS England’s Local Health and Care Record Exemplars programme.
The concept was born out of a desire to provide health and care staff with better and faster access to vital information about the person in their care and provide citizens with access to their information, encouraging them to be more involved in looking after their health.
Interweave partners with a number of Integrated Care Systems which means Interweave is accessed by a diverse range of organisations from NHS Trusts, Local Authorities, Social Care, Ambulance Services and GP practices.
